1. Acceptance of Terms
By accessing careswaps.com or engaging CareSwaps services, you agree to be bound by these Terms of Service. If you do not agree, please do not use our services.
2. Services Provided
CareSwaps provides care transition coordination services including, but not limited to:
- Readiness assessments and resource guides (free tier)
- POA and MDPOA documentation verification assistance
- Facility identification and coordination
- Medical records transfer coordination
- NEMT (Non-Emergency Medical Transportation) booking assistance
- Room setup coordination (Room Replicator)
- 30-day post-move monitoring check-ins
CareSwaps is a coordination and logistics service. We do not provide medical care, legal representation, or financial planning.
3. Fees and Payment
Self-Guided (Free): Access to resource guides, readiness assessment, and educational content is provided at no cost.
Full Coordination ($1,999): A one-time flat fee covers all coordination services from initial assessment through 30-day post-move monitoring. Payment is due before services commence. The fee is non-refundable once active coordination has begun, except as described in Section 4.
Facility Subscription ($399/month): Facilities are billed monthly. Subscriptions may be cancelled at any time with 30 days' notice. No refunds are issued for partial months.
All payments are processed by Stripe. CareSwaps does not store payment card information.
4. Refund Policy
For Full Coordination clients: If you cancel within 48 hours of payment and before we have begun active coordination activities, you are entitled to a full refund. After active coordination has commenced, the fee is non-refundable. If CareSwaps is unable to complete coordination due to circumstances within our control, we will issue a prorated refund.
5. No Referral Fees
CareSwaps does not accept referral fees, commissions, or payments from care facilities in connection with family placements. Our facility subscription program is a separate, flat-fee service and does not influence family placement recommendations.
6. Your Responsibilities
- You represent that you have legal authority (POA, guardianship, or personal agency) to make care decisions for the individual on whose behalf you are engaging services.
- You agree to provide accurate and complete information necessary for coordination.
- You understand that final placement decisions rest with you and the receiving facility, not CareSwaps.
- You agree not to use our services for any unlawful purpose.
7. Limitation of Liability
CareSwaps coordinates logistics and documentation. We are not responsible for the clinical decisions, care quality, or outcomes at any care facility. To the fullest extent permitted by law, CareSwaps' liability is limited to the amount you paid us for services.
8. Governing Law
These Terms are governed by the laws of the State of Colorado. Any disputes shall be resolved in the courts of Larimer County, Colorado.
9. Changes to Terms
We may update these Terms from time to time. Continued use of our services after changes constitutes acceptance. We will post the updated Terms at careswaps.com/terms/.
