One flat fee covers everything we do to make your swap happen.
We search our entire network to find a facility with your loved one's care needs, timeline, and location preferences. We handle initial outreach and qualification.
We find another family going in the opposite direction with compatible timelines and facilities. This creates the bilateral exchange that makes swaps possible.
We coordinate discharge paperwork, medical records transfers, admission applications, and HIPAA compliance between both facilities. You stay informed every step.
We assess your loved one's transport needs and connect you with vetted NEMT or medical transport partners. We coordinate pickup dates and times with both facilities.
We lock in exact swap dates so both moves happen simultaneously. No waiting for your loved one's bed to open—both transfers occur together.
Throughout the process, you have a dedicated point of contact at CareSwaps. Questions? We're here. Problems? We solve them.
One-time coordination fee. That's it. No additional charges, no hidden costs, no percentage of any transfer.
We match your loved one with the right transport option for their medical and behavioral needs.
Non-emergency medical transportation for stable patients who can sit upright and don't need medical monitoring during the move.
For patients needing professional medical care during transit: oxygen, IV support, cardiac monitoring, or behavioral oversight.
If it's right for your loved one's condition, you can transport them yourself with CareSwaps' coordination and facility support.
Sarah wanted to move her mother from a nursing home in Texas back to Colorado to be closer to family. Michael's family wanted to move his father from Colorado to Texas to be near his other children.
Tom's mother needed memory care that only existed in Denver, 3 hours away. Elena's father had lived in Denver memory care for 2 years but wanted to move to his hometown in the mountains.
Jennifer's grandmother was on a waiting list for the "best" assisted living facility in their state—2+ year wait. CareSwaps found a family looking to leave that exact facility.
Margaret's father was in a nursing home but had deteriorated so much he now needed rehabilitation care. The nearest rehab center had a 6-month wait. Margaret felt trapped.
Once we've found a match and both facilities agree, most swaps complete within 4-8 weeks. This is much faster than traditional placement, which can take years.
Before your swap is finalized and both facilities have locked in dates, you can cancel with no penalty. Once dates are locked, cancellation terms depend on your agreement with the receiving facility.
We'll keep searching for 90 days. If we don't find a match in that time, we'll discuss next steps with you. Your $1,999 fee is refunded if no match is found.
No. We help you evaluate options—vetted NEMT partners, medical transport, or family transport—and coordinate with the facilities. You decide what's right for your loved one.
Absolutely. CareSwaps is HIPAA compliant with BAA-signed service providers. Your loved one's medical information is encrypted and only shared with facilities and transport partners when necessary.
In theory, yes. But the swap needs to work both directions. We're looking for another family wanting to go the opposite way with compatible timelines. The more flexible you are on facility specifics, the faster we find a match.
You do. CareSwaps handles matching and coordination, but transport companies are independent. You'll contract with them directly and pay their fees. This keeps CareSwaps out of the transport business.
We pre-screen all families and lock in swap dates only when both sides are fully committed. If something changes, we have backups in our network to minimize disruption.
If your loved one cannot consent to their own transfer, you'll need an MDPOA or guardianship to authorize the swap. We recommend having this in place before you register. Read our full MDPOA guide →
